Well most stories focus on cross dressing not transgender people. While still wearing clothes on the other gender they mostly identify as the sex they are born with but like cute things or something.
The trap-thing is not really good, but I think the translations are partely to blame.

If someone is transgender it can show as a child. I think it is no harm in a social transition. I mean who cares about someones cloths or interests and such.
Surgery is on a different level and should not be performed on minors and as far as I know is not really happening.
