They chose to continue being together without looking each other in the eye. One day, there is this disaster or attack (something happened) and bakugo got really injured, he ran to or ran into kirishima, memories of him flashing through his mind, they met eyes and forgot each other..

Omg that's so sad! The tried so hard to still remember each other and yet, at the end, Kacchan dies forgotten. I'm crying
