What the title means exactly is that the female lead falls for the male lead at first sight and that the male lead isn't what you'd consider "conventionally attractive", he's someone who's often misunderstood based on his looks, he is someone many consider intimidating, or it's unexpected for some other reason.
It's not exactly "unexpected" that she'd fall for him so fast since he had just rescued her from a creep, plus he's hot as fuck. But, I'm marking it as that considering their vast size/height difference, and the fact that he has a pretty intimidating aura, which I'm assuming isn't something she's very used to. They did both seem to fall in love at first sight (she even mentioned that).
