One of the three MCs is a trans girl. This story contains physical, emotional, and sexual abuse.
This is a short, cute manga about the life of a married trans woman. This is autobiographical.
A character is revealed to be a trans guy in chapter 20. There are also lesbian, bisexual, and gay characters. This is completed, which is a bit sad because it didn't get to reach its full potential.
This has a sidestory about Yuki, the adult trans woman from Horou Musuko.
One of the protagonists can switch from being afab to amab, which is more of a classic gender bender trope. However, they do not have a set gender identity, so I'm including this as enby rep.
One of the characters, Itaru, is described as a crossdressing boy, but a lot of the context implies that they are actually trans.
I didn't expect to get to add this to the list and am very excited about it. The girl in chapter 16 is trans. The art of this manga is stunning too.
This manga is from the early 80s and rather dated. Hibari is obviously a trans girl, but this manga tends to be transphobic and a wee bit racist.
I'll update this if I read more than the first page, but the mc is a trans woman.
Same case as the last entry. I haven't read this yet, but the MC is a trans girl.

Is is about intersex people, which could qualify as trans.
also homunculus is a great book overall but it felt like the two main characters were secretly in a shounen ai and then it went into body dysmorphia and closeting to meet expectations and then they seem transition and is accepted. its was honestly such a cool surprise to find canonical trans rep that was handled well in a surreal horror-ish manga.
danganronpa has a trans character that wasn’t handled as well as i wanted it to be, but better than initially expected. there’s tons of djs but im pretty sure there’s an official adaptation.
