This is a 4-koma manga about a trans woman. Just as a bit of background, the term 男の娘 (otokonoko) is used instead of 男の子 (otokonoko). The latter is the term for a boy. The kanji 娘 means daughter, so 男の娘 is a pun that's frequently used in crossdressing and transgender communities in Japan. Additionally, lot of the language surrounding the LGBT community in Japan is either a discriminatory or clinical, so many Japanese people opt to use English loanwords instead.
There is a trans girl, but she isn't introduced until over 100 chapters in and dies. It was nice to have some casual representation (that's not the butt of a joke) in a classic survival game shonen horror manga.
Alluka is trans. Everyone who says otherwise can die angry lol.
There is a trans man in chapters 10&11. The rep is generally positive, but I feel conflicted about it because of the presentation of intrinsic and sexually dimorphic neuroanatomical differences and how they relate to the manifestation of gender as a form of self identification. Ie the whole female body, male brain mumbo jumbo. The manga steps on some controversial modern neuroscience, but it's also a comedy manga from the mid 90s. It ain't that deep, bub.
The mc is a trans girl and there is an adult trans woman side character. The representation leaves a good bit to be desired, but it seems be trying its best as a comedy manga from 2004. It feels like your typical early 2000s shoujo romance with trans characters as a little pinch of spice.
The mc is a trans boy. This is just a yomekoma
One of the mcs is a trans girl. This manga is a mess and a half; it had a plethora of things it wanted to talk about (being trans, suicide, sexuality, AIDS, kidnapping, child abuse, growing up, and way too much more) but kept forgetting that it's only 6 chapters. It has good intentions, but doesn't properly execute its messages. I thought it was needlessly angsty and unrealistic. Nice unreliable narrator tho!
Same creator as Wonder Egg Priority.
This is just a short and sweet lesbian romance with a trans woman love interest.
This is oneshot about a trans woman deciding to go to Thailand to get bottom.
This is a very cute autobiography by a trans woman. I guess it's probably important to note that legally transitioning in Japan requires a diagnosis of GID (which is no longer a medically recognized term), forced sterilization, and bottom surgery. Global human rights organizations have generally spoken out against this, but it looks like it's been improving a bit in recent years.

Is is about intersex people, which could qualify as trans.
also homunculus is a great book overall but it felt like the two main characters were secretly in a shounen ai and then it went into body dysmorphia and closeting to meet expectations and then they seem transition and is accepted. its was honestly such a cool surprise to find canonical trans rep that was handled well in a surreal horror-ish manga.
danganronpa has a trans character that wasn’t handled as well as i wanted it to be, but better than initially expected. there’s tons of djs but im pretty sure there’s an official adaptation.
