Sakura doesn't outright say they're a trans girl, but Amane is a trans guy. Very cute t4t romance but apparently dropped by the author
Lily, one of the idol girls, is trans. However, this manga doesn't have the chapter revealing this translated yet. Just watch the anime. It came first, and the manga is a heavily abbreviated version of the story.
One of the characters, Grell, is heavily implied to be a trans woman. I've seen the term okama used to describe them, which is a pretty offensive phrase that I'd probably translate as the f-slur. A YouTuber called Hazel made a good analysis video on Grell that sums up most of my complaints/thoughts. I'm not personally interested in this series, so I'm not well versed on its idiosyncrasies.
The mc is a trans woman with two other trans friends. This is smutty het manga, but it's pretty pure. The way she experienced gender dysphoria and euphoria was more nuanced than I expected, like how she worried about the size of her hands. I've worried about the same sort of things lmfao
4-koma about a lesbian trans woman. Literally just one 4-koma
The FL is a trans girl, but this manga is total scum and awful. It's a harem manga with the FL lead given the "trap" treatment, met terrible stereotypes, and constantly misgendered. This is included in the list bc I want you to know what stuff that is constantly recced is abysmal.
One of the MCs in chapter 2 is a trans woman. I thought the story was kinda cute and described the pain of not passing, but it also leaned a little too heavily into the okama stereotype.
I'm really not sure how a manga with a trans girl MC by Oshimi Shuzo of all managaka slipped past me, but here we are. I'm saving reading this one for a rainy day. Knowing the author, this is probably going to be a phycological thriller with borderline uncomfortable sexuality.
I've been recced this one several times over my better part of a decade collecting trans manga, and I genuinely can't get past the first chapter because of the trap treatment. It's also a little too ecchi for me
I've been hesitant adding this to the list even tho there is very much a trans girl character because there's only one chapter up currently, and she hasn't even been introduced yet lol.

Is is about intersex people, which could qualify as trans.
also homunculus is a great book overall but it felt like the two main characters were secretly in a shounen ai and then it went into body dysmorphia and closeting to meet expectations and then they seem transition and is accepted. its was honestly such a cool surprise to find canonical trans rep that was handled well in a surreal horror-ish manga.
danganronpa has a trans character that wasn’t handled as well as i wanted it to be, but better than initially expected. there’s tons of djs but im pretty sure there’s an official adaptation.
