In almost every omegaverse I've read, the baby comes out the ass because the uterus is attached to the colon/rectum. In a few stories, male omegas always have c-sections. Honestly depends on author. Some get C-sections I believe where others have them come out from the ass. Also, I only read one thing where giving birth greatly affected heath and it was just M-preg not omegaverse. reply
