I think you need to stop watching/reading shounen only. Of course SHOUNEN will be set in Highschool, since that's the demographic that they are targeting. When you look at Josei/Seinen shows/comics, the MC will always be a grown man/woman and the setting is usually in a office or workplace. That started out so good, the concept is really great and we had the potential of getting a queer character in a Shonen jump manga. Omegaverse world is really adjustable so it highly depends on the author. There are omegaverses while Alphas can get pregnant but there are those where they cannot. I have read some omegaverse where the MC had omega x omega parents, so it entirely depends on the author.
