You're right. Had he died, his actions would have been even more cruel, and he would have died feeling sanctimonious at having "selflessly" let her go, when it would have only made her more miserable. What a complete scumbag.
he ignored her made her insecure and broke her heart in the worst way.... even if he was sick before he knew he was sick he was still ignoring her, prioritizing the beautiful mother and daughter over her, and instead of trusting her and relying on her he chose to hurt her and traumatizing her... one she would never trust anyone in a relationship again and two if he had died she would feel even worse... he was super selfish
