They never really say it bluntly that he died but it is kind of obvious that he died since watanuki owned the shop at the very end for more than 100 years.
Well he did died, but the mangaka didn't say it, but Watanuki did said that the current Doumeki looks like his great-grandfather..

I was so sad! Cuz, sorry if you don't like, but I'm a yaoi fan-girl, and for me, it's DoumekixWatanuki! It was so cute when Doumeki swears that he won't let Watanuki die alone, which he then changes to never be lonely since he realizes that they can't die together. Their relationship is very cute, since they have each other to rely on. It broke my heart when I realized that the current Doumeki is the old one's great grandson. :'(
