because in this world, alpha and omegas are really rare, and betas i believe cannot have kids. overall it is REALLY hard to have kids (hence why the omega male cat which is stronger due to being male, an omega that can have kids, and on top of that a CAT which often has multiple babies at once is SUPER crazy insane in this world)
we can also very quickly assume that people grow up very quickly in this world. hence why their first born grew really fast and ended up basically being the fated mate of the doctor.
the doctor had to have sex because that is literally his job. if omegas do not have an omega, and they want children, they go to clinic where they can become impregnated basically so they don't quite literally die out as a race. it's not weird because it's literally his job. (i understand it but still dont like it much.)
