I think the main issue being that it was a forced gender change which is the real issue. And there was never an option to remedy it. The MC was basically r-wordered into accepting that this is his new identity.
If they wanted to go this way in a fluffy type bl then yeah I think that no one should worry about the gender transition, but this case is different since where on the darker section of the genre.
By fluffy bl I mean as a trans man being with another man, and it not being forced on them for someone's selfish personal wishes
