Well, as with all the homophobia presented in this manga (and some sexism too), I think that was just another way to show how hateful this society/time period is. I dont think that the author is homophobic, racist, or sexist; just that the characters they have written are.
When she called the woman "negro" in chaper 2, by heart literally dropped to my ass... And I immediately wanted to stop reading.
