Nah you're right. I think it's just easier for them to say "they were brutally tortured" rather than show realistic consequences of it in order to fit their narrative. It's just a side chacteristic as means to make the FL more pitiful while the focus is mostly on the plot. It'd be cool if more manhwas had a stronger touch of realism to them

This is why I like The Male Lead’s Adopted Daughter. She actually looked malnourished, and even once cleaned up and healed, she’s still smaller than all of her peers. The rest of it, I guess is explained away by her magic abilities.
