So in the last one the sister has a split personality. One male( a brother ) and one female ( a sister and her physical self) since it was forbidden love she felt she locked her male self "away" but thinks that it's actually the doc who does it when in reality the doc is the brother but she is so far off that she doesn't know that.....I felt I made this quite confusing in an effort to simplify
