I think it's pretty clearly about like death and the meaning of life. Wataru was probably suicidal, or at least not entirely satisfied with life. It seems like he'd gone through a lot of shit in his life and struggled to find meaning. He died during the crash at the start, whether he was intentionally trying to kill himself or not. Then the rest of the story is basically him sorting through his regrets and desires, trying to find meaning and convince himself and others he wasn't suicidal or anything like that (the people of the kingdom disregarding life was a reflection of himself to a certain degree, but of a part that he hates). He wanted to go on a trip across america with his friend (I wouldn't be surprised if he actually did die during high school, like the nurse said), regrets about what happened to his ex (I think akari dying was also a reflection of that), a bunch of other stuff. His biggest regret was probably leaving his mom behind tho. It seemed like she was struggling a lot through her life, and Wataru had been traumatized as a result of that. He didn't want to leave his mom behind and relied on her notes to get through things.
I could go on and on ughhh and there's still so much I didn't get. This manga was so good I'm so glad I read it...
