The duchess is the duke's wife. The duke is the FL's step-son, the duchess is her step- daughter in law. She was married to the former duke in writing but they never met I think. As to why the duke and duchess look alike, i think its just how they are drawn.

in webtoon I'd get why they make people have various colors of hair and eye color to present associating the characters as related, but I guess its just the common color, just like how many of us have the same hair and eye color but unrelated
