So far, I haven't had this problem... it may be that I'm new to this website since I only made my account yesterday but I've marked at last 60 manga as "already read" and whenever I sign in to check on that manga or click it on the homepage, it's marked like that.

So far, the stuff I have noticed looks like a new upload, but then it turns out to be something I've already read. Idk, it's weird haha.
Hmm well I'll keep an eye out for it and I'll let you know if I see something like that. If it is happening then it's understandable since it's a reupload and things change with the upload... like extra chapters and so on so maybe it automatically gets removed from your list?
There're duplicate uploadings because many manga have various names: Japanese, English, Korean, Chinese, etc.

As ArdenWinter said there're quite many. I have lost count how many time I report admins to fix it, not to mention others report. HA+ (≧∀≦)
