I'm kinda annoyed bwat is using persian word Padishah for king but then switch to Italian version of an arabic word (Sultana) for Queen instead of Shahbano

I think about it this way
The king is from the kingdom of the sun so he is the padishah
But the queen is from another country hence why the different in wording

I think bwat did not pick the titles with their word origins in the mind. An example coming to mind is that in Ottoman empire the ruler was Padishah and Sultana title was given to their spouses and later their daughters/Princesses. So maybe Bwat just went way lol
