The Angel of Judgement's stigmata (the one Pavel has) comes in pair with Saint Zeon's stigmata (the one Sion/Siennas currently has). It only awakens when Saint Zeon's appears. That's how Pavel knew Sion got reincarnated, because he couldn't feel his stigmata beforehand.
When activated, it creates an event so powerful it got compared to "the day of judgement", thus the naming. It basically kills a lot of people (the earthquake or whatever it is that happened before Sion died). It also triggers Saint Zeon's stigmata's "miracle" (remember the wish Sion made before he died), something like a catalyst.

I believe it’s not the stigmata that kills a lot of people, but rather that killing a lot of people and then dying that activates the true purpose of the angel of judgement stigmata- hence the story of the previous king who ordered his angel to kill a bunch of people and then die on top of their bodies. also, we see in Pavel’s flashbacks that he had stormed the great temple and slaughtered a ton of the guards/militia before he found Sion & killed himself, therefore activating the “Great Miracle”
Also if my interpretation is correct, it seems like the stigmatas allow them to have some kind of power? Separate from the main purpose of the stigmata?? we know that Pavel used his powers in the Heo-Polinekan war, which would have been before Sion was back, and it seems like Sion/Siennas was suspicious that someone (likely Pavel) had used their powers to tamper with his memory. + also Ferhi in one of the most recent chapters mentions the stigmata bearer’s “powers” which seems to support this theory
I still have lots of questions about the stigmata’s and what exactly the “powers” they can wield with them are but that’s what I’ve gathered so far ╥﹏╥
