Lol,from what I remember the modern world was the hallucination and he was crazy in the martial world, the medicine that man was giving him kinda gave him awareness but he’s been lost in his mind due to a sickness and he thought the medicine would cure him so he’d be in the modern world but the sickness made him makw that up, each person he was seeing in the modern world is technically somebody from the martial world and his gf if the white haired girl but I don’t think he pieced that together. The man gave him a pill that cured him and he no longer was able to go to the modern world. It’s a bit confusing to type out

It’s not an assumption but it’s what the story led mc to believe, like I said it’s all wonky and while he might be from the martial woukd it’s been to many things happening that points to the fact the modern world may also be real. It’s a physiological story after all it’s meant to mind fuck you. In the beginning he believed he was from the modern world but it turned out he’s from the martial world but evidence show both could be real and while the him that’s in the martial world might be actual from there the modern world could be a mirror world like I said almost everybody had a real counterpart.
