I have a theory that the medications reduced the heat temporarily. Pilsang never skipped the meds at all, so the side effects might show later on after a while of not taking it.
Maybe the medications only dulls out the hormones that triggers his heat (does it act like an antibiotic or an antidepressant? I'd like to hear people's theory about this). While it is unlikely, I also think the side effects might be abrupt and chaotic. Because if he were to feel the side effects significantly little by little, He would've sensed that something was weird from when he stopped taking the meds entirely before.
I don’t think the medication works like antibiotics or antidepressants. It seems more similar to contraceptive pills, which are meant to regulate or suppress hormonal cycles. These pills have hormones that directly affect the body's hormonal balance and are often used to control irregular periods, lessen menstrual pain, or even stop heat in animals.
And since Pilsang was given the Sianacin drug which was originally used on animals, it’s likely that the medication works in a similar way, by altering hormonal levels to prevent heat.
Regarding side effects, they could typically be sudden and unpredictable rather than gradual, since hormonal suppression can cause abrupt changes in the body's functions. Any adverse effects might appear quickly and be quite chaotic. However, since Pilsang hasn't experienced any symptoms at the moment, and we're considering the possibility of delayed effects, there's a chance that stopping the medication suddenly could trigger a delayed but intense and disorderly response, with symptoms emerging some time after cessation. Moreover, because his body has been heavily altered by the drug, his physiology might react in a way that causes a rebound or an even more severe heat as a response, as well as leading to serious delayed consequences.

Okk- alright, so this was initially my response to one comment, but since I've seen a hella lot of comments repeating the same thing, I’ll just copy and paste it here :
The medication is NOT what triggers the heat.... In fact, it has been shown to significantly reduce the severity of Pilsang's symptoms and the frequency of his heat episodes. He also went from being unconscious and knocked out during his heats to being fully conscious and able to remember everything that happens during them. I truly don't understand how people can believe the medication is causing the heat when, in reality, it has helped him so much.
Also, I'm not saying it's completely safe or without side effects, since it's still an experimental drug, but it definitely is NOT causing the heat ! Or else, it doesn't make sense !
Him stopping the medication abruptly could, on the other hand, be related to the delay of his heat, and as a consequence, it might come back twice as badly when it does return...
