Here's my take on it (the misunderstanding).
Perhaps it's not completely a misunderstanding at all. Nathan doesn't feel dumb, he probably knew that Alex was lying at the time but because he refused to fess up the truth (alternatively "the whole truth", in his eyes). But didn't and instead kept lying to him, so Nathan probably was just like "fine if that's how u wanna play it" and left, carrying on this grudge.

For the first point I kind of find it interesting but let's say in an alternative universe Alex did in fact inflict that upon Nathan for whatever fucked up reason, it would be possible that he would later regret it and want to back out of it once it got down to it out of guilt or because of superficial reasons like being held responsible
It's possible he didn't want to be there himself, but rather sell him out for things like cash etc rather than their own pleasure

But it still feels really out of left field for me at least, especially for Nathan’s character. If it was my bestie and I in that situation, even if I really thought he did it, I’d question what happened. Because when you had a bond for so long and something like that happens I feel like you question everything and from him it felt like he was willing to accept it at face value.
Especially since he wanted to be a police officer, I feel like he would especially try to figure everything out but he just didn’t. I feel like him not looking into it at all just didn’t feel like something Nathan would do.

I like this point and thought on it some more and I feel like if that were the case Alex would immediately explain himself away of give a half assed apology but he didn’t. He was incredibly quick to take incriminating blame. Maybe it’s cus I watch too much true crime but for me that feels like someone incentivized him to take the blame. But then again if u want to be an officer and you're as smart as Nathan, I feel like he would know that.
Yeah your not wrong about that. But honestly people can be very unpredictable even if you think your family or close for that matter, unfortunately.. so it's not too big of a surprise I guess when it boils down to it that he thought wrong

Even without the context we have as readers, a lot of elements of this event were incredibly fishy.
Firstly, the fact that Alex ran to him despite allegedly being the person who wanted him attacked isn’t adding up. If he wanted to see Nathan suffering in pain he would’ve been there at the start. He organized this with the out come of Alex being in pain, so if he’s at the scene, it’s because he wants to witness it. So why did he show up, running at full speed during the middle of it despite already knowing when and where his attack would happen? How do you set up a time and place according to your schedule and be late to your own event?
Secondly, why call someone who you know won’t be able to pick up multiple times? If Alex knows that Nathan will be beaten and treated so horribly he can’t pick up, why call Nathan back to back? These people already said Alex did it so why would he expect Nathan to pick up if he thinks Nathan would now hate him? To confirm that he was injured and his people did the job? If that was the case after the third time calling with no answer, why not call the people you hired, since they will be physically well enough to answer the phone.
Thirdly, Nathan constantly points out how Alex can’t control his emotions well at all. He knows Alex likes him due to his behavior and facial expressions. So when Alex called him frantically multiple times, ran to him arriving there physically exhausted, and evidently looks distraught and unwell, Nathan didn’t pick up on any of that at all? I fully understand that Alex said he did it, but even so his behavior was so frantic that it’s honestly hard to not be suspicious of what he said he did. Not only did it not align with his character, it didn’t make sense because previous Alex already said he’d only have relationships with an omega, why not just turn him down by saying that again? This escalation is completely out of character. Maybe if he didn’t know Alex well this would make sense but they’re best friends.
