I'm thinking maybe Mr. Shin did something to his wife which caused her to be in a coma? Perhaps, upon finding out she cheated, he tried to drown her in rage almost killing her..
Maybe the "Alter Ego" or "Clone" title refers to Mr. Shin? He may have a more dark and dangerous side than what we expect.
