Everyone is just mad. At this point I don't see how the people in the whole community are any different from the ones with that random disease? All have some mental problems, it seems to show more outside in people who also got(?) physically stronger - but not like any of the "normal" people seem to be sane either.
And maybe that is the point. Everyone is already sick, going crazy, and it's pointless like the Sisyphus bolder. All of them are crazy hounds.
(Sidenote: It's really hard to imagine any satisfying ending for this story how it's right now, and I start to suspect that is also the point.)
