I’m asking before reading , do the mains at some point get together and act like a couple or is it a: “you question everything until the end but at least there’s good plot”

The main story is R15, the side stories are R19.

And the answer is yes they do and by then it is fantastic.

Yeah, it's very slow burn. They're not officiall dating until after the great dungeon is defeated, though they slowly get closer the whole time.
