No, that's not why they are breaking up, no one was cheating. They literally just want 2 very different things in a relationship, and neither thing is bad, but in that aspect there is no way they'd both be happy if things continued. And yes, Takatora is the one with the hysterectomy.
As for the opening scene, why they are shirtless, it is because Osuzume had gotten drunk fell on top of Takatora, and puked on their clothes.
