I am not sure, if it applies to this story, but in most stories the secondary gender is usually a somewhat newly discovered thing. Now that I think about it, that is a great question, since in this story, the secondary gender has been present generations before... But it could be that omegas get same reproductive organs as women do, but it's kind of fused with the anus (I mean, they even get wet down there)... That's at least how I've always imagined it, but I am no medical expert, so I purely rely on the imagination... ╮(￣▽￣)╭
Every authors omegaverse is different but there are two popular trends in every if not most universes, either secondary genders existed since the beginning of humans or that they existed near the last one to two centuries (1900s-2000s). In this case it hasn’t been confirmed whether or not omegas can give birth through their anuses maybe Joshua wanted to have a C-section.
You also forgot to mention that he wasn’t born an omega! Originally, Joshua was born a beta and into manifested as an omega when he was over-exposed to Chase’s extreme Alpha pheromones, MAYBE omegas who manifest late can only give birth through C-section meanwhile omegas since birth can do both C-section and natural birth but this is just my take on it

I was imagining it in a way that maybe it can be similar to our world's history, I guess? I completely forgot Joshua's secondary gender change, lol. Thank you for telling me, that adds a lot of insight into this discussion.
It's also a bit concerning when I imagine the post partum of naturally birth. How torn are their anuses? How hard would it be to move? As for c section, it's hard too, although it doesn't look as uncomfortable as the latter in a surface level.

This makes me wonder (this is completely random btw), in the past did the male omegas give birth through their anus? Because it wasn't until the 1800s did c section was recorded. In the first place, this is a fictional world so maybe they had to resort to c sections in the ancient times of omegaverse. The big issue is their clinical procedures, anesthetic, and the sanitation. Does that also mean that male omegas giving birth are prone to death? If we're talking in this sense, maybe omegas and female betas giving birth was already a death sentence back then. Idk just came up my mind
