Correct me if I read it wrong but the grand duke is not connected by blood to the current emperor. He is the child of the empress from another man. Though since he is now considered as the emperor's step-son they are kinda like cousins by law sooo..
Ooooh okay, so it *was* mentioned that he's not actually the Emperor's son. That's where I was confused since I didn't recall it being stated. But if it is, then (relatively) no issue then, lol

And... I'm confused by these family relations.
Kaela's father was the "late Emperor's illegitimate son". Late Emperor meaning the current Emperor's father? So Kaela's father is the Emperor's brother...
Meaning she and the Duke are cousins???
The only other thing I can think of is that the Duke was the child of the Empress she had with another man before marrying the Emperor, but I honestly don't remember that being stated in the story
