Cesare had to kill multiple Eileen’s to get back to the reality where she’s alive, that’s why he confused this one with the past memories. He had once already killed Eileen in that bedroom in the brick house, and it had rained that day too (it rained many times whenever Eileen died or he found her dead) thats why when he was abruptly awoken by Eileen he strangled her and tried to choke her. Obviously he understood that was the real Eileen and stopped himself, even cut his own palm multiple times to make sure that he felt pain meaning this was reality. After that Eileen fell asleep cause she had used too much strength already, while at first fighting (it shouldve been visible in the last panels of the last chap) and then when Cesare was close to choking her completely. Cesare carried Eileen and brought her back to the grand ducal house cause he couldn’t remain in the brick house bedroom anymore, and that’s when he decided to rearrange his memories about all the past times so that he wouldn’t make such a mistake again
