Jihwan has always been like this, from the start. But without his meds it’s obviously going to get extreme.

But I don’t blame Jihwan for getting off his meds. Being forced to take medication (by doctors he hates) since he was a kid; Jihwan doesn’t know a life that’s not medicated. Getting off his medication must be freeing in both a mental sense and also control sense. He has one less thing controlling him now, and with Jihwan, this is just the start, since he likes being in control of everything.

Another thing to note in this chapter is how Jihwan selects what he wants to hear. We know Jihwan struggles with appropriate social behaviour, and this is just one example of that. He has never been taught how to deal with conflict, so he just ignores it.

Last thing to note: Jihwan has said he would never lay a hand on Soo-young multiple times throughout the story. I don’t think it’s a coincidence that him pulling soo-young by his hair, only happened after he stopped his meds. I think Jihwan attempts to live by a set of morals, but he unknowingly pushes them, or when he does knowingly push them, he frames it as a loophole so he’s not “technically” going against his morals. But now without his meds…I wonder if he’ll give up his morals entirely.
