I think its most likely black magic cuz his symptoms not only physiological but also psychological or maybe he mentally/psychologically fragile thats why it appears like that (im talking about when he tried to ask ishimon- for example- )
But i cant really tell that he really is weak mentally cuz the past him was strong mentally and have a strong personality so idk
