Sherlock sharing smoke with a skull maybe to symbollize that he still hungs up on the past (i like the idea that sherlock is smoking with the "dead" lord of crime, because while i think he felt guilty about killing somebody, i wouldn't think he would smoke with milverton either), and william holding a rose petal in his mouth and the flower in his hands....
