We've seen him blow up when Renji goes too far time and time again and the author has shown us that Sakuma is capable of making his own choices. Sakuma said it himself too that he can power through it which is why he CHOOSES to leave it in, even if he did regret it at the end. But he rightfully tells Renji not to do it again.
