The thing that bother me is that MC is shown to be abused when he is still a teenager, so the parents attitude didn’t change after the accident. So why did the parents suddenly change their attitude all of the sudden. Hopefully the plot reveals that.
I think the fake brother is some sort of reality bending entity who probably got obsessed with mc back when he had that accident and kinda made his wishes come true. His family suddenly became loving.
