The omega is sexually assaulted multiple times—by both his stepbrother and the alpha's brother—once they find out he is an omega; the alpha (main lead) also assaults him (as a form of "punishment") and knots him. Neither the protagonist's family nor the main lead's brother faces any kind of punishment. The omega's life is complicated and sad. Throughout the novel, he suffers significant physical abuse, including from his stepbrother. They end up together with a "happy ending." You should be mindful of your mental health if you decide to continue reading the manhwa.
