From what I've heard from ppl that read the original novel... Him not bringing up the miscarriage is an intentional storytelling tactic; it's supposed to relate to how women and mental illness were treated in this era. Mental illness means madness and insanity, it doesn't mean depression or anxiety that needs to be treated. We know that pregnancy and miscarriage taken astronomical tolls on the body and mind but this era and these people don't. Because if you bring attention to it, you are practically admitting that the victim of the miscarriage is mad or insane. In addition I think Ezekiel genuinely doesn't grasp the consequences of miscarriage and that's intentional. Men in this era (heck even nowadays) don't understand the mental consequences of a miscarriage so it's tragically realistic for Ezekiel to be brushing over it.
I don't say this to change your mind btw, if you're disappointed then you're disappointed, I just wanted to bring up some interesting discussion points I saw in other comments ^-^

Guys I've never been this disappointed in a manhwa in my life The build-up all the way to him regaining his sight was near perfect, it has me gripping to my chest and strumming my heartstrings. However, ever since these latest chapters where he figured out that Rizanne was Rose, everything just got worse. Ever since then, he hasn't mentioned her miscarriage, never actually tried to talk to her (with pen and paper) and most of all I dont think he gave an apology worth accepting. I just hope that all these issues will be addressed soon but I doubt it'll actually happen
