I came across this one a while ago and it randomly popped into my head again and it's driving me nuts that I can't remember the title or find it anywhere.
it's smutty, romance between stepsiblings. the girl is older but tiny and has some sort of health condition. the brother returns home for some reason to help with....something? the girl I think works in a book store, possibly.
but the first time they sleep together, or something adjacent to that, the girl like has some sort of issue related to her health condition and starts bleeding everywhere??? it was crazy.
