Basically - there are six genders - every combination of Male/Female with Alpha/Beta/Omega. Omega men and all women can get pregnant. Omegas have heat cycle (works like menstrual cycle) which emitates pheromones so that Alphas crave to impregnate them. There are also soul bounds - if an Alpha bites Omega on their cycle on the neck, they are bounded and Omega's pheromones will only work on this Alpha and his body will reject other Alphas (or people in some mangas)

And Beta are like the normal one that doesn't get affected by the Omega. They are the one that has no bound. And all of the alpha/beta/omega can get pregnant.

I think being able to get pregnant varies from manga to manga since I've seen both versions (but I encounterned more of the ones where it's only omega that can get pregnant)
