I don't think Dohyun is actually happy at the moment. He's mentally broken. He lost all will to live because of the trauma and went into an unresponsive state, but then he started remembering Sungho and decided to seek comfort in those memories, because he truly loved Sungho when they were young and it was the only time he's ever felt safe and happy. So he's basically pretending adult Sungho is young Sungho, which is possible because for the moment Sungho is also reverting to their old dynamic and letting Dohyun take the lead. Dohyun smiles, teases Sungho, and acts playful because that's what young Dohyun would do, not because he's suddenly okay with everything that happened.
On the other hand, Dohyun probably recovered from his injuries because it would be boring for him to be in bed for weeks unable to move or have sex. Comics logic.
