Looks does not change anything! I have seen ukes and semes who look like babies but their age is in the green zone, there is even a manga when an two adults *prp* do a baby kink...
Well personally I don't read any manga between adult/minor (so thanks to this comment giving me the heads up to skip this manga, haha).
But that said, looks (as in the drawing style) do matter for some people. Subconsciously it's easier to handwave and forget about a character's age if they look like an adult, and conversely it can be uncomfortable to read manga where one of the characters looks super young even if they're supposed to be an adult.
Also I have to correct you on the "13 is the age of consent in Japan!" because that's a popular bit of misinformation. 13 is the age of consent under *federal* law, but every prefecture has local laws that put the age of consent higher than that. Nobody is (legally) having sex with 13-year-olds in Japan.
