Proabably because it shown in cover clearly it is Okumuracest so people who don't like incest don't click and read. But sometimes in manga you can't tell if it is incest or not so people starting to read. That's why I think.
It's literally a twincest fanfiction. This isn't a surprise reveal or something for shock value, they are twins and they are fucking and that is the premise of this manga.
By the fact that they are blood related twins? I see people get really bothered by other mangas where the step brothers get together but nobody is talking about how these two are blood related
