I have not read the Novel but based on this Manhwa: the last Empress test was changed due to Rosemond interferring.

Rosemond check for the Empress candidate's fertility and chose amongst them the infertile and made her Queen.

Just that alone, how powerful Rosemond must be in the Palace to be able to do that.

Rosemund is smart, I’ll give her that. If the Queen is infertile, it will cause a scandal amongst the gentry and aristocracy and it will either make the King divorce her (if it’s allowed) or he will have to knock up Rosemund and make her child the successor to the throne.

Actually , she is not infertile . Her result was changed by the king but I don't know why he changed her medical result specifically.I think he did this because he is well aware that rose is not a person who is suitable to be mother of a nation and he is kinda serious in regards to his responsibilities as a king.Later on in the story, Rose does use the medical result of Patricia being infertile to be appointed as imperial consort/concubine.

I feel like a part of the emperor, somewhere in his small, small, small ass brain he had some doubts about Rosemund and her potential of being the Queen. He still loved her but I think the King’s council didn’t approve because he needed a partner of some status, (not sure if Rosemund had no status before the King gave her title as Baroness) and marrying someone out of love wasn’t seen as ideal. I guess his council’s words got to him little by little and planted a seed of doubt inside him, but this is my theory.
