Fumichi must have depression or some other mental issue, which prevents him from taking agency in his own life that is not self-destructive. I feel like this manga is to be read from Fumichi's perspective, where he himself is only half aware of what is happening with his life. As if he has practically no energy to spare for his wellbeing, while he still retains some awareness, i.e. he wishes to gift the stroller. I would say, though, that the author overplayed how low Fumichi has fallen with the whole "trash" smell... But anyway, I would say this is not really romantic in any way, rather, might make you empathise with mental patients...
