I've thought about this ever since I was 14 lmao and many years later, I've arrived to the general conclusion that:

1. No, female omegas most probably don't get periods bc their heat basically acts as an agent for mating and pregnancy
2.No, male omegas also don't get periods, basically works on the same principle as aforementioned above
3. Yes, in the many omegaverses that I've read and come across, female alphas do have a vagina, yes they CAN get pregnant but it's extremely difficult for them to conceive since their primary method of reproduction is using their dick lmao
4. I don't know if female alphas get periods though, it's highly debatable bc female alphas themselves are shown extremely rarely, let alone having a special focus

But they need an egg to be fertilised for pregnancy to occur

Ye some omegaverse gives alphas and omega superpowers like regeneration and healing while others are realistic but they follow the same structure of mating

Female alphas function in the exact same way as male alphas do. Primary gender doesn't make a difference. So i.e, yes, they do experience ruts and can inflate a knot.
A female alpha is what you'd call a "futanari", she has all the usual female reproductive systems but with the addition of a penis (but futas don't have a vulva do they? That's the main difference). Still, depending on the author, there can be slightly anatomy changes on how their penis and testicles present. For instance, some authors prefer both testicles and penis hanging out externally just like men with their vulva just beneath them OR all of their "alpha" parts are placed internally, wherein only their penis pops out of their vulva when they're horny.
To answer your last question, het axo couples are also pretty much dependant on how the author prefers it but generally, the omega takes it up the ass (=・ω・=)
