I believe he loved her, they got divorced as his way of protecting them. There are different types of love, maybe he just never felt sexual love towards his wife.

They loved eachother and they have daughter. Doc left them as a way to protect them from everything that can happen due to his connections with police. (One of previous cases put his family in a great danger). In recent chapter his ex wife regrets she wasn't strong enough to stay with him despite the danger and she looked pretty hurt once she noticed doctor is thinking about someone else.
Doc stated he never was with any men before nor he ever felt attraction to one. Until he met MJ. So it's more likely that their marriage wasn't out of convenience (as it is in many stories where the MC is probably gay, because God forbid for Bisexual or any other sexuality to exist in BL, and he's just married to satisfy his parents)
