So basically I'm 99% sure I've seen this manga on mangago. It's novel name is Continuation of the Dream in Another World. The novel's summary is: On a day when he was 14, Toki had a dream in the hospital. He dreamt about a prince from a different world. An everyday life of a handsome young man with blond hair and blue eyes for three years. It was the same for the other party as well. Eventually, Toki arrived in the other world.
