It's called being intersex. It's a genetic mutation (usually) so there's a lot of different kinds but basically you develop a combination of sex characteristics from both sexes. In his case he has an outward male appearance but has an internal uterus and apparently ovaries bc he got preggers. roughly 1% of the human population has some kind of intersex condition.

It's rear but real where on the outside you can look like a man but on the inside you have both organs you can Google it

Well, us intersex people do exist, but we do not get pregnant through our ass. There are many different intersex conditions though and they all present differently, and sometimes not at all. Some of us can get pregnant and some can’t, but majority, if not all, of us who do have a vaginal opening/uterus. If you don’t, then in some cases your eggs can be taken and used for an IVF through your partner or other consenting party. If you have both testes and a uterus and want to become pregnant then you might need oestrogen therapy, or remove testosterone from your body, or it can be hard to become pregnant or give birth.

Mpreg as a concept exists in the real world, but not really in the same sense that omegaverse biology works.

And trans men can also conceive children if they want to.
