Based on some that I've read so far it's actually the first alpha to smell/sense the omega in heat that bites the omega... since alphas lose their control they bite the unwilling omega (eventhough omegas in heat want nothing but to be bleeeeped by an alpha during that time - mindset is to reproduce) . So I think rather than fate it's more like an instinct between the two. haha But as the other comment said, rules can be changed accdg to the author

Ive read one manga where the omega was already bitten by an alpha before, then later on meets the one he loves (not the alpha that bit him), resulting to everytime he'll go in heat and have bleeep with his lover I think it's really painful for his part (psychological) he's crying as if going crazy because it's not the alpha who bits him. (eventhough he trulllly loves his lover)... so in this case yes, can still go doki doki though have some effects during heat

The bite is a way of Alphas marking territory. If an Alpha bites an Omega during the Omega's heat, the Omega can't have sex with anyone else (or they can, rather it'll just feel disgusting). It doesn't matter to the Alpha though, and the Omega can still go into heat.

Depends on the author (again) but in Romantic Juntou he felt bad doing it with somebody else only during his heat. Which is nice, they can continue their life even with a mark
