For Komiya, at 18, to still have a sticks-and-bones sort of appearance (when he didn't in middle school) probably just means puberty wasn't entirely nice to him. Plenty of non-athlete guys look like skeleton twigs into their early twenties. The amount you have to eat to maintain a decent body weight at that time would be absurd and unhealthy at any other point in life.
Yeah, the realistic misunderstandings here are worrying, but kind of cute at the same time. Nishi calling Komiya a fairy was adorable too.
